<b>Background</b> Angiotensin converting enzyme inhibitor-induced angioedema is a major cause of angioedema-related hospitalizations each year. Common swelling predilection sites include the face, periorbital skin, lips, pharynx, larynx, and tongue. Involvement airway structures could lead to obstruction ultimately death if not treated promptly. A 62-year-old man presented rural clinic with massively swollen tongue that obscured visualization retropharynx mildly lips. The patient stated he had been taking lisinopril for many years never episode similar this. was initially administered epinephrine, antihistamine, steroids, Tranexamic acid. second dose acid 30 min later followed by close monitoring. Seven hours after first administration, near-complete resolution discharged. Angioedema treatment still variable hospitals in different settings may be able take advantage available resources treatment. cheap widely antifibrinolytic used treat angiotensin angioedema, especially more specific therapeutics are unavailable.
